Who do I send the completed FMLA forms (or) other medical information

Employees’ can upload any medical documentation into their Absence Plus  leave request page or email the medical documentation to Betty E. Ochoa and Amanda Carpenter through the leave management shared email [email protected]

Can I apply for FMLA to care for a family member?

Under the Family and Medical Leave Act (FMLA) a family member is defined as a:
•    A Legal spouse
•    A Parent of:
o    Biological
o    Adopted
o    Foster
o    Step
o    Who stood in loco parentis for the employee when a child under the age of 18*
•    Child who is
o    Biological
o    Adopted
o    Foster
o    Step
o    Legal ward under the age of 18
o    Child of a person (employee) standing in loco parentis*
o    An adult child who is incapable of self-care**

Maternity leave of absence follows the City of Detroit’s pay-code cascade for “Self”

Returning to work after continuous FMLA or other medical leave of absence

Provide a return to work letter from your doctor indicating the date you can return to work and if there is any restriction or not, at least a week before your return to work to Leave management Absence Plus  [email protected] and your Employee Services Consultant. Call ph# 313-224-8584 .

How should I report Job Related Accident / Injury?

Please inform your supervisor as soon as possible. You or your supervisor will complete a First-Aid report and will have you complete an "Employee Accident or Disability Report". What is my username?

After submitting a leave request to Absence Plus you will receive a username from Absence Plus. 

What is my temporary password?

After submitting a leave request to Absence Plus you will receive a temporary password from Absence Plus.

Can I request a leave of absence from my mobile/cell phone?

No, Absence Plus does not have a mobile app.

Will my medical documentation be secured once I load it into Absence Plus?

Yes, only the leave management team will have access to your medical documentation which will remain privilege and confidential.
