The Detroit Health Department Family and Community Health goal is to empower and engage individuals, families and communities through health and wellness information and activities. Using a coordinated client centered approach to build initiatives, programs, education and trainings we will address health from a life course perspective to improve quality of life for all who live work and play in the city of Detroit.
These are the programs we offer:
Infant Safe Sleep: Working with numerous partners we facilitate safe sleep classes throughout Detroit at; churches, community agencies, social service agencies, workforce development offices, health centers, hospitals and schools etc. The classes focus on teaching the A,B,C’S (Alone, on the Back, in a Crib and in a Smoke free environment) of safe sleep practices. We also distribute Pack N’ Play portable cribs to mothers, who qualify and are in need of a safe sleep environment for their baby. In addition we have made Pack N Plays available to individuals who cannot participate in a group class however is willing to come to a one on one educational session.
Fetal Infant Mortality Review-Case Review Team (FIMR-CRT) gathers monthly to review abstracted clinical cases combined with information gained during maternal home interviews with families who have experienced the death of an infant less than a year old. We are seeking to identify patterns of care, maternal and clinician behavior and potential gaps in services that could possibly be addressed to prevent future infant deaths.
Father’s Forward Initiative will equip Fathers with the necessary skills, resources and linkages to play an integral part in their child’s life from pregnancy and beyond. Fathers Forward will focus on- making organizations “Father-Friendly”; training men from the community to mentor fathers and expecting fathers; and creating a “brotherhood” for men in the City of Detroit.
Community Health Workers Training: DHD will be the central cross training hub for CHW’s in the city of Detroit. Working in collaboration with MiCHWA (The Michigan Community Health Worker Alliance-statewide Hub) DHD will offer both educational and career paths for Lay Health Workers, Seniors and community leaders who we will recruit to actively work towards making their community healthier.
Health Outreach Promotion and Education: The Detroit Health Department will focus on providing the community with information and education addressing various health issues in order that residents might live well. Education and information will focus on: Smoking cessation, diabetes prevention and management, exercise and obesity prevention, heart health, blood pressure and control, stress management and mental health.
