  • Get discounts at select local businesses

  • Available to ALL Detroit Residents
  • Not a replacement for a Michigan ID or driver’s license 
  • Cannot be used for driving or travel

  • Collect documents totaling 300 points.

How to Make an Appointment:

  • Call 313-774-5388
  • Then apply in person Monday through Friday, 9:00 a.m. to 5:00 p.m. at:
    • Detroit Health Department, 100 Mack Avenue, 48201
    • Patton Recreation Center, 2301 Woodmere, 48209

Guidelines

  • No age limit, but under 18 needs a guardian
  • Apply in person only
  • Bring original documents, so no copies or electronic files
  • False information is illegal
  • Card is valid for 2 years
