Monday, Wednesday, Friday (closed City holidays and furlough days)
9:00, 10:00, 11:00 a.m. & 1:30 p.m. (Arrive at least 10 minutes before to fill out application)
Location: Basement of the Main building.
Enter through the Taylor Ave. entrance go to basement and follow signs to registration window (past the pharmacy)
Permit Cost:
$10.00 food service workers
$5.00 food service volunteers (religious, charitable, non-profits)
$5.00 duplicate card
