Application for Registration of Rental Housing

The following step-by-step instructions are intended to assist the applicant in properly completing the "Application for Registration of Rental Housing" form. It is important that the instructions are followed closely to ensure that the application is processed in a timely manner. The applicant must present the following supporting documentation, along with the completed application form to:

The following requirements must be met:
(1) If property is Section 8, a notarized "Affidavit for the Issuance of Certificate of Compliance for Section 8 Rental Housing" form must be submitted along with the application. See below for link to form.
(2) The inspection report from the Detroit Housing Commission.
(3) Current identification (driver's license, state-issued photo ID or passport).

Do not fill out the "Partnership or Corporation" section, if this is not a partnership or corporation that is registered with the federal government.

Do not fill out the "Resident Agent for Corporation" section, if this is not a partnership or corporation.

Do not fill out the "Management Firm" section, if there is no management firm


Step 1:    The residential rental property owner or an authorized representative must check the appropriate type of rental housing registration. Select either "Initial Registration" or "Change of Ownership". Please check only one type (per form). Both boxes are not be checked.

Step 2:  
 The address (street number and name) of the residential rental property must be provided.

Step 3:    The name of a cross street must be provided.

Step 4:    The name of a second cross street must be provided.

Step 5:    The appropriate "Type of Dwelling" must be circled. If "Other" is circled, a description must also be provided.

Step 6:    The number of Dwelling Units Rented must be provided as follows:

The number of apartments/residents to be rented
The number of sleeping (bedrooms) rooms

Step 7:    Check either "Yes" or "No". If "Yes" is checked, the residential rental property owner or authorized representative must click here to obtain a copy of the "Affidavit for the Issuance of Certificate of Compliance for Section 8 Rental Housing" form. Complete the form and attach a copy of the inspection summary report from the Detroit Housing Commission. The number of Section 8 housing units must also be provided.

Step 8:    Print the full name of the residential rental property owner or authorized representative.

Step 9:    Provide the complete address (include street number and name, city,state and zip code) of the residential rental property owner or authorized representative. This address must match the information listed on the owner's or authorized representative's valid driver's license, state-issued photo ID or passport.

Step 10:    Provide the telephone numbers (include the area codes) of the residential rental property owner or authorized representative. List the home, business, cell and fax numbers.

Step 11:    If this is a partnership or corporation that is registered with the federal government, provide the name, address and phone number of the business. If applicable, enter information for up to two partners or officers.

Step 12:    If this is a partnership or corporation that is registered with the federal government, and there is a residential agent for the corporation, provide the agent's name and title.

Step 13:    If this is a partnership or corporation that is registered with the federal government, and there is a residential agent for the corporation, provide the agent's complete home address (include city and state), and telephone number. If applicable, enter information for up to two residential agents.

Step 14:    If there is a management firm for this residential rental property, provide the name of the firm.

Step 15:    If there is a management firm for this residential rental property, provide the name of the residential manager or caretaker. Note: Enter information for up to two residential managers or caretakers.

Step 16:    If there is a management firm for this residential rental property, provide the complete address (include city, state and zip code) of the residential manager or caretaker.

Step 17: 
   If there is a management firm for this residential rental property, provide the telephone number (include area code) of the residential manager or caretaker.

Step 18:    The residential rental property owner or an authorized representative must sign the application. Do not print the name in this field.

Step 19:    The residential rental property owner or an authorized representative must provide a title.

Step 20:    Provide the date the application was signed by the owner or authorized representative.
