Plumbing Contractor License Register Or Renewal

Step-By-Step Instructions - Application for Renewal or Registration of Contractor License (Plumbing) The following step-by-step instructions are intended to assist the applicant in properly completing the "Application for Renewal or Registration of Contractor License (Plumbing)" form. It is important that the instructions are followed closely to ensure that the application is processed in a timely manner. The applicant must present the following supporting documentation, along with the completed application form to:

Supporting Documentation Required:

(1) Current state of Michigan contractor's license, and
(2) Current identification (driver's license, state-issued photo ID or passport).

Note: If applying for or renewing a wrecking, sign and awning erector, and/or window washer license, do not fill out the "Supervising Employee" section of this application.

Step 1:
To register or renew a PLUMBING contractor's license, check Plumbing under the "Type of License or Registration" section. Applicant must only check one type of license (per form). A separate application must be submitted for each additional license request.

Step 2:
Applicant must provide the name of the business (company). This name must match the business name on applicant's state of Michigan contractor's license. Applicant must present valid state of Michigan contractor's license and current identification (driver's license, state-issued photo ID, or passport).

Step 3:
Applicant must provide complete business telephone number (include area code).

Step 4:
Applicant must provide the business address (street number and name). Address must match the business address on the state of Michigan contractor's license.

Step 5
: Applicant must provide city in which the business is located. City name must match the city name on the state of Michigan contractor's license.

Step 6
: Applicant must provide state in which the business is located. Must be “MI” for Michigan.

Step 7:
Applicant must provide business zip code (if known, enter the 9-digit zip code; for example “48206-1234”). Zip code must match the zip code on the state of Michigan contractor's license.

Step 8:
Applicant must provide his/her address (street number and name). Address must match the address listed on the applicant's current driver's license, state-issued photo ID, or passport.

Step 9:
Applicant must provide city in which he/she lives. City name must match the city name listed on the applicant's current driver's license, state-issued photo ID, or passport.

Step 10
: Applicant must provide state in which he/she lives. Must be "MI" for Michigan and match the information listed on the applicant's current driver's license, state-issued photo ID, or passport.
Step 11: Applicant must provide valid zip code (if known, enter the 9-digit zip code; for example “48206-1234”). Zip code must match the zip code listed on the applicant's current driver's license, state-issued photo ID, or passport.

Step 12: Applicant must provide a valid email address.

Step 13:
Applicant must provide his/her valid driver's license number, state identification number, or passport number.

Step 14:
Applicant must provide his/her date of birth. Date of birth must match the date of birth listed on the applicant's current driver's license, state-issued photo ID, or passport.

Step 15:
For security purposes, applicant must provide only the last 4 digits of his/her social security number.

Step 16:
Applicant must print his/her full name. The name must be printed and not signed in this field.

Step 17:
Applicant must provide his/her signature. The name must be signed and not printed in this field.

Step 18:
Applicant must provide his/her complete telephone number (include area code).

Step 19:
Provide supervising employee's full address (street number and name).

Note: Supervising employee is the licensed person holding the business license.

Step 20:
Provide supervising employee's city.

Step 21:
Provide supervising employee's state. Must be "MI" for Michigan.

Step 22:
Provide supervising employee's valid zip code (if known, enter the 9-digit zip code; for example “48206-1234”).

Step 23
: Provide supervising employee's valid email address.

Step 24:
Provide supervising employee's valid driver's license number, state identification number, or passport number.

Step 25:
Provide supervising employee's date of birth.

Step 26:
For security purposes, provide only the last 4 digits of the supervising employee's social security number on the application.

Step 27:
Supervising employee must print his/her full name. The name must be printed and not signed in this field.

Step 28:
Supervising employee must provide his/her "Signature". The name must be signed and not printed in this field.

Step 29:
Supervising employee must provide his/her complete telephone number (include area code).

Step 30:
If the business name identified in Step 2 is a partnership or corporation, provide the full name for a partner or officer. If the business name identified in Step 2 is not a partnership or corporation, skip this section. Enter information for up to two partners or officers.

Step 31:
If the business name identified in Step 2 is a partnership or corporation, provide the title for a partner or officer.

Step 32:
If the business name identified in Step 2 is a partnership or corporation, provide the home address for a partner or officer.

Step 33:
If the business name identified in Step 2 is a partnership or corporation, provide the city for a partner or officer.

Step 34
: If the business name identified in Step 2 is a partnership or corporation, provide the state for a partner or officer.

Step 35
: If the business name identified in Step 2 is a partnership or corporation, provide the zip code for a partner or officer. If known, enter the 9-digit zip code; for example “48206-1234”).

Step 36
: Note: This section of the form must be signed in the presence of a Notary Public. Applicant must provide his/her signature and title. Please be advised that the BSE&ED department has notaries on-site.

Step 37:
Form must be notarized by a Notary Public. Applicant must present the completed application and current identification (driver's license, state-issued photo ID or passport) to a Notary Public, and sign it in his/her presence. The Notary Public must verify the signature in the space provided on the form. Please be advised that the department has notaries on-site.
