Application for Renewal or Registration Contractor

The following step by step instructions are intended to assist the applicant in properly completing the Application for Renewal or Registration of Contractor License form. It is important that the following instructions be followed very closely to ensure that the application is approved in a timely manner. Applicant must present the following supporting documentation along with the complete application form to the Buildings, Safety Engineering and Environmental Department, Room 408, Coleman A. Young Municipal Center, Two Woodward Avenue, Detroit, Michigan 48226: (1) State of Michigan Contractor's License and (2) un-expired identification information (driver's license, state-issued non-driver photo ID or Passport). Note: These instructions may be printed to guide you through the process.

First Time Registration with City of Detroit: Require Electrical Contractor's/Master Electrician's License application AND Registration of Contractors License application.

Step 1:    Applicant must check the appropriate Type of License or Registration. Applicant must only check one type (per form). You can not check two boxes, you must utilize another form, when applicable.

For a Electrical renewal or registration select the checkbox for "Electrical"

If applicant holds only a Electrical Contractor License, he must hire a Master Electrician License holder and complete Business, Applicant and Supervising Employee Sections

If applicant holds both a Electrical Contractor License and Master Electrician License, complete only Business and Supervising Employee sections. Leave Applicant section blank.

Step 2:    Applicant must provide the name of the business (company). Business name must match the business name on the State of Michigan Contractor's License. Applicant must have their current State of Michigan Contractor's License, unexpired identification information (driver's license, state-issued non-driver photo ID, or passport).

Step 3:    Applicant must provide the complete business telephone number (10 digit phone number is required which should include the area code).

Step 4:  
 Applicant must provide the full Business Address (Street Number and Name). Address must match the business address on the State of Michigan Contractor's license.

Step 5:    Applicant must provide the city information for the business.

Step 6:    Applicant must provide the state information for the business.

Step 7:    Applicant must provide valid zip code information for the business. You must at least provide the five digit zip code (if known, enter the 9-digit zip code, for example "48206-1234")

Step 8:    Applicant must provide their full address (Street Number and Name). Address must match the applicants address on their valid Driver's License, State of Michigan ID, or Passport.

Step 9:    Applicant must provide their city information. Must match the applicants city information on their valid Driver's License, State of Michigan ID, or Passport.

Step 10:    Applicant must provide their state information. Must match the applicants state information on their valid Driver's License, State of Michigan ID, or Passport.

Step 11
:    Applicant must provide valid zip code information for the business. Must match the applicants state information on their valid Driver's License, State of Michigan ID, or Passport. You must at least provide the five digit zip code (if known, enter the 9-digit zip code information; for example "48206-1234").

Step 12: 
   Applicant must provide a valid email address information.

Step 13:
    Applicant must provide their driver's license number, state identification number, or passport number. Number must match the supporting identification information.

Step 14:
    Applicant must provide their date of birth information. Date of Birth information must match the supporting identification information.

Step 15:    Applicant must provide the last 4 digits of their social security number.

Step 16: 
   Applicant must "Print" their full name.

Step 17: 
   Applicant must sign the application. Do not print your name in this field.

Step 18:    Applicant must provide their complete telephone number (10 digit phone number is required which must include the area code).

Step 19:    Supervising Employee must provide their full address (Street Number and Name) information.

Step 20:    Supervising Employee must provide their city information.

Step 21:    Supervising Employee must provide their state information.

Step 22:    Supervising Employee must provide their zip code information. Must at least provide the 5-digit zip code. If known, please provide the 9-digit zip code (Format: 48204-1234).

Step 23: 
   Supervising Employee must provide their valid email address information.

Step 24:    Supervising Employee must provide their un-expired driver's license number, state identification number, or passport number.

Step 25:    Supervising Employee must provide their date of birth.

Step 26:    Supervising Employee provide only the last 4 digits of their social security number on the application. Note: For security purposes only provided the last four digits.

Step 27:    Supervising Employee must "Print" their full name on the application.

Step 28:    Supervising Employee's "Signature" must be provided on the application.

Step 29:    Supervising Employee's employee telephone number (10 digit phone number - which includes the area code) must be on the application.

Step 30:    Applicant must provide the full names of the Business Officers or Partners, if the Business Name in the Business Section above is a Partnership or Corporation.

Step 31:
    Applicant must provide the titles of the officers/partners.

Step 32: 
   Applicant must provide the home address information (Street Name and Number) of the officers/partners.

Step 33:    Applicant must provide the city information for the officers/partners.

Step 34: 
   Applicant must provide the state information for the officers/partners.

Step 35:    Applicant must provide the zip code information for the officers/partners. Must at least provide the 5-digit zip code. If known, please provide the 9-digit zip code (Required Format: 48204-1234)

Step 36: 
   Applicant must provide the full names of the Business Officers or Partners, if the Business Name in the Business Section above is a Partnership or Corporation.

Step 37:    Applicant must provide the titles of the officers/partners.

Step 38:    Applicant must provide the home address information (Street Name and Number) of the officers/partners.

Step 39:    Applicant must provide the city information for the officers/partners.

Step 40:    Applicant must provide the state information for the officers/partners.

Step 41:    Applicant must provide the zip code information for the officers/partners. Must at least provide the 5-digit zip code. If known, please provide the 9-digit zip code (Required Format: 48204-1234).

Step 42: 
   Applicant's must sign and date the application in the presence of a Notary Public. Please be advised that the BSE&ED department has notary's onsite.

Step 43: 
   Form must be notarized by a Notary Public. Applicant must take the completed application form and your unexpired U.S. Driver's License (or State ID) to a Notary Public, sign application form in his/her presence, and ask him/her to verify your signature on the application form in the space provide in this section. Passport may be utilized as a form of identification also.
