Request for Business License Form

The following step by step instructions are intended to assist the applicant in properly completing the Request for Business License form. It is important that the following instructions be followed very closely to ensure that the application is approved in a timely manner. Applicant must present the following supporting documentation along with the complete application form to the Consumers Affairs Department, Business License Center, Suite 105, Coleman A. Young Municipal Center, Two Woodward Avenue, Detroit, Michigan 48226 or call 313-224-3179.

    (1) You will be required to submit the fee(s) to cover the cost of processing the application and is non-refundable. (Payable by check or money order) make payable to the "Treasurer, City of Detroit. Oter forms of payment methods accepted are "Visa", MasterCard", Debit Card, or Cash.
    (2) Un-expired identification information (driver's license, state-issued non-driver photo ID or Passport and
    (3) Registered Business Name with the Wayne County Clerks Office/Assumed Name Division -or- (4) The Registered Corporation, Limited Partnership, Limited liability Corporation Name with the Michigan Department of Consumer and Industry Services. Note: These instructions may be printed to guide you through the process.

Step 1:    Applicant must provide the full Address (Street Number and Name). Address must match the address on the Business/Corporation Documents.

Step 2:    Applicant must provide the city information for the business.

Step 3:    Applicant must provide the state information for the business. Must be "MI" for Michigan.

Step 4:    Applicant must provide valid zip code information for the business. You must at least provide the five digit zip code (if known, enter the 9-digit zip code, for example "48206-1234").

Step 5:    Applicant must provide the name of the business (company). Business name must match the business name on the State of Michigan License. Applicant must have their current State of Michigan License, unexpired identification information (driver's license, state-issued non-driver photo identification, or passport).

Step 6: 
   Applicant must provide the name of the business (company). Business name must match the business name on the State of Michigan License. Applicant must have their current State of Michigan License, unexpired identification information (driver's license, state-issued non-driver photo identification, or passport).

Step 7:    Applicant must provide a Yes or No answer to the question.

Step 8:
    Applicant must provide the name of the business (company). Business name must match the business name on the State of Michigan License. Applicant must have their current State of Michigan License, unexpired identification information (driver's license, state-issued non-driver photo identification, or passport).

Step 9:    Applicant must provide the full Address (Street Number and Name). Address must match the address on the Business/Corporation Documents.

Step 10:    Applicant must Select type of Business from list.

Step 11:    Property Owner name must match the Owner name on the State of Michigan Deed. Applicant must have their current State of Michigan License, unexpired identification information (driver's license, state-issued non-driver photo identification, or passport).

Step 12:    Property Owner must provide the complete telephone number (10 digit phone number is required which should include the area code).

Step 13:    Property Owner must provide the full Address (Street Number and Name). Address must match the address on the State of Michigan Deed.

Step 14:    Property Owner must provide the city information for the location .

Step 15:    Property Owner must provide the state information for the location. Must be "MI" for Michigan.

Step 16:    Property Owner must provide valid zip code information for the location. You must at least provide the five digit zip code (if known, enter the 9-digit zip code, for example "48206-1234").

Step 17:    Property Owner name must match the Owner name on the State of Michigan Deed. Applicant must have their current State of Michigan License, unexpired identification information (driver's license, state-issued non-driver photo identification, or passport).

Step 18:    Property Owner must provide the complete telephone number (10 digit phone number is required which should include the area code).

Step 19:
    Property Owner must provide the full Address (Street Number and Name). Address must match the address on the State of Michigan Deed.

Step 20:  
 Property Owner must provide the full Address (Street Number and Name). Address must match the address on the State of Michigan Deed.

Step 21
:    Property Owner must provide the state information for the location. Must be "MI" for Michigan.

Step 22
:    Property Owner must provide valid zip code information for the location. You must at least provide the five digit zip code (if known, enter the 9-digit zip code, for example "48206-1234").

Step 23:    Business Owner name must match the Owner name on the State of Michigan Deed. Applicant must have their current State of Michigan License, unexpired identification information (driver's license, state-issued non-driver photo identification, or passport).

Step 24:    Business Owner must provide their date of birth information. Date of Birth information must match the supporting identification information.

Step 25: 
   Business Owner must provide the titles of the officers/partners.

Step 26:  
 Business Owner must provide the full Address (Street Number and Name). Address must match the address on the State of Michigan Deed.

Step 27:    Business Owner must provide their city information. Must match the applicants city information on their valid Driver's License, State of Michigan Identification, or Passport.

Step 28:    Business Owner must provide their state information. Must be "MI" for Michigan. Must match the applicants state information on their valid Driver's License, State of Michigan Identification, or Passport.

Step 29:    Business Owner must provide valid zip code information for the business. Must match the applicants state information on their valid Driver's License, State of Michigan ID, or Passport. You must at least provide the five digit zip code (if known, enter the 9-digit zip code information; for example "48206-1234").

Step 30:    Must provide only the last 4 digits of their social security number on the application. Note: For security purposes only provided the last four digits.

Step 31:  
 Business Owner must provide their driver's license number, state identification number, or passport number. Number must match the supporting identification information.

Step 32: 
   Business Owner must provide the complete telephone number (10 digit phone number is required which should include the area code).

Step 33:    Business Owner name must match the Owner name on the State of Michigan Deed. Applicant must have their current State of Michigan License, unexpired identification information (driver's license, state-issued non-driver photo identification, or passport).

Step 34:    Business Owner must provide their date of birth information. Date of Birth information must match the supporting identification information.

Step 35:    Business Owner must provide their date of birth information. Date of Birth information must match the supporting identification information.

Step 36:    Business Owner must provide the full Address (Street Number and Name). Address must match the address on the State of Michigan Deed.

Step 37:  
 Business Owner must provide their city information. Must match the applicants city information on their valid Driver's License, State of Michigan Identification, or Passport.

Step 38: 
   Business Owner must provide their state information. Must be "MI" for Michigan. Must match the applicants state information on their valid Driver's License, State of Michigan Identification, or Passport.

Step 39: 
   Business Owner must provide valid zip code information for the business. Must match the applicants state information on their valid Driver's License, State of Michigan ID, or Passport. You must at least provide the five digit zip code (if known, enter the 9-digit zip code information; for example "48206-1234").

Step 40:
    Must provide only the last 4 digits of their social security number on the application. Note: For security purposes only provided the last four digits.

Step 41:
    Business Owner must provide their driver's license number, state identification number, or passport number. Number must match the supporting identification information.

Step 42: 
   Business Owner must provide the complete telephone number (10 digit phone number is required which should include the area code).

Step 43:    Business Owner name must match the Owner name on the State of Michigan Deed. Applicant must have their current State of Michigan License, unexpired identification information (driver's license, state-issued non-driver photo ID, or passport).

Step 44:    Business Owner must provide their date of birth information. Date of Birth information must match the supporting identification information.

Step 45:    Business Owner must provide the titles of the officers/partners.

Step 46:    Business Owner must provide the full Address (Street Number and Name). Address must match the address on the State of Michigan Deed.

Step 47:    Business Owner must provide their city information. Must match the applicants city information on their valid Driver's License, State of Michigan Identification, or Passport..

Step 48:    Business Owner must provide their state information. Must be "MI" for Michigan. Must match the applicants state information on their valid Driver's License, State of Michigan Identification, or Passport.

Step 49:    Business Owner must provide valid zip code information for the business. Must match the applicants state information on their valid Driver's License, State of Michigan ID, or Passport. You must at least provide the five digit zip code (if known, enter the 9-digit zip code information; for example "48206-1234").

Step 50:    Must provide only the last 4 digits of their social security number on the application. Note: For security purposes only provided the last four digits.

Step 51: 
   Business Owner must provide their driver's license number, state identification number, or passport number. Number must match the supporting identification information.

Step 52: 
   Business Owner must provide the complete telephone number (10 digit phone number is required which should include the area code).

Step 53:    Applicant telephone number (10 digit phone number - which includes the area code) must be on the application.

Step 54:    Applicant Fax telephone number (10 digit phone number - which includes the area code) must be on the application.

Step 55:    Applicant must provide their driver's license number, state identification number, or passport number. Number must match the supporting identification information.

Step 56:    Applicant must provide their driver's license number, state identification number, or passport number. Number must match the supporting identification information.

Step 57:    Applicant must provide the full Address (Street Number and Name). Address must match the address on the Business/Corporation Documents.

Step 58:    Applicant must provide the type of location agreement from "Own" , "Lease", "Lease Agreement".

Step 59:    Give Details

Step 60:    Applicant must provide number of Business License Required.

Step 61:    Applicant must provide seven digit License Code (if known).

Step 62:    Applicant must provide Bill Cycle (if known).

Step 63:    Applicant must provide Business License Type.

Step 64:    Applicant must provide fee associated with the License

Step 65:    Account number provided by Business License Office

Step 66:    Entered by Intake person

Step 67:    All lines of License Type applied for.

Step 68:    Approval Signature

Step 69:    Applicant's must sign and date the application in the presence of a Notary Public. Please be advised that a notary's is located in the building.

Step 70:    Form must be notarized by a Notary Public. Applicant must take the completed application form and your unexpired U.S. Driver's License (or State ID) to a Notary Public, sign application form in his/her presence, and ask him/her to verify your signature.
