Senior Citizens 911 Profile Agreement
The Detroit Police Department is firmly committed to providing professional service to all residents. However, in an effort to improve our service delivery model for our senior citizens (65 years of age or older) the Detroit Police Department will be implementing a new service:
Senior 911 Profile. This service will allow our senior citizens to have their individual profiles stored in our 911 database and provide critical care information to the 911 call-takers in the event of an emergency situation. This should improve the call taking, dispatch and emergency response process.

Please read the agreement in its entirety, fill out all of the required fields marked with an asterisk *, and complete any additional information that you would like to include. Download the Agreement
The profile can be faxed to (313) 596-1610. Please include a copy of your Michigan State I.D. or Driver's License.
The profile can also be hand delivered to any Detroit Police Department Precinct/District. However, if someone else is dropping off the profile for you, you must include a copy of your Michigan State I.D. or Driver's License with your profile . Blank copies of the Senior Profile are also available at the following Detroit Police Precincts/Districts:
